4. Authorization & Consent

I authorize Mighty House Inc. (Computer Store), an Education Service Provider, to help me prepare and submit my family's Wyoming ESA (Steamboat Legacy Scholarship) application. I remain the applicant: I will review the application and personally sign or attest wherever required. Where the program permits authorized assistance, Mighty House may transmit or submit the prepared application for me. Where the program requires me to submit or sign personally, Mighty House will guide me to do so. I am at least 18 and am the parent/legal guardian or a person having legal control or charge of the child. Mighty House is not the State of Wyoming and not the WDE; it cannot approve applications or guarantee funding. I may revoke this authorization before submission by calling (307) 314-9485. *

How far may Mighty House go?

I consent to Mighty House Inc. storing the information on this form, contacting me (email, phone, or text) about my family's ESA application and ESA-eligible products and services, using my information to measure and improve outreach, and using non-sensitive intake signals to identify and assist other households who may benefit from school-choice help. Mighty House does not sell family or student information and does not send student documents, birth dates, SSNs, or sensitive child-level details to ad platforms.
